These terms govern your use of this site, online enrollment and the BELONG member portal, offered by Ultrahealth. By using them, you accept them. Membership rights and obligations are also governed by the membership agreement you sign when you enroll; if they differ, the agreement prevails.
The membership is not insurance
BELONG is a direct primary care agreement under section 624.27 of the Florida Statutes. It is not health insurance, is not subject to the Florida Insurance Code and does not meet the minimum essential coverage requirements of the Affordable Care Act. We recommend keeping health insurance for hospital stays, emergencies and specialists.
Emergencies
The site, portal and messages are not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
Fees, charges and renewal
- The membership fee is charged in advance at the frequency you choose (monthly, quarterly, semiannual or annual) to the card you registered, and renews automatically until you cancel it under the agreement.
- Services not included in your plan are charged at the member price of the current fee schedule you accepted. Fee schedule prices may change with prior notice.
- Plan change and new household member requests stay pending until the clinic reviews them and are not charged until approved.
- If a charge is declined, we may suspend benefits until the payment is resolved.
Your account
- You are responsible for keeping your password confidential and for all activity on your account.
- You must give us truthful information and keep it up to date.
- The primary member can view and manage the information of household members that the law allows.
Acceptable use
You agree not to use the site for unlawful purposes, not to try to access other people’s accounts or data, and not to interfere with its operation.
Intellectual property
The site’s text, logos and designs belong to Ultrahealth or its licensors and may not be used without permission.
Limitation of liability
General information on the site does not replace a visit with a health professional. To the extent the law allows, we are not liable for indirect damages arising from the use of the site or its temporary unavailability.
Governing law
These terms are governed by the laws of the State of Florida and the applicable federal laws of the United States.
Changes
We may update these terms. We will post the new version on this page with its date and, if the change affects your membership, we will notify you in advance.
