When you finish we email you a link to activate your account. Use it to sign in to the portal and the app to see your card, book visits and check your benefits.
Receive care and save
Show your card at the front desk and pay member prices.
Services included in your plan have no extra cost; everything else is paid at the member price published in the fee schedule.
